The small ends have a 4mm center gem with three 2mm side gems on both sides of the center gem
The medium ends have a 5mm center gem with four 2mm side gems on both sides of the center gem
The large ends have a 6mm center gem with four 2.5mm side gems on both sides of the center gem
This is just a threadless end, the gauge is referring to what gauge threadless barbell you are putting this end onto.
