For septum spikes - Gauge size refers to the center notch. Septum spikes are going to be made one size larger, but the notch in the center will be the proper gauge.

I got this pincher in my regular circular barbell size, which is pretty snug. It fits a little smaller/ more subtle than I had anticipated, but it was a mistake on my end, so if you want the ends to stick out a little more, maybe get a diameter up. Still looks super cute and subtle!
